5. Supplemental Teaching License Application
6. Evaluation Request for Licensure through the Supplemental Pathway
This evaluation may be requested only by teachers who hold a supplemental license and are requesting a state equivalent review for full licensure. Teachers who are enrolled in an approved program of preparation at a college or university would continue to follow the college's program. An evaluation would not be needed.

To be used by teachers who had only one opportunity to pass the Praxis III/Entry Year Program during the life of the two-year provisional license.
10. Temporary License

This application may be used only by employing schools/districts when requesting full-time temporary licenses for pupil services, military science, 12-hour teaching permits and 40-hour STEM School teaching permits. This application must be initiated by an employing Ohio school district.
11. Duplicate or Correction Request
Interactive Application! You can complete this application on the computer. When finished, print it, sign it and mail it to the address on the application.
This application may be used to request a copy or a correction of your current license.
12. Substitute License - Long-Term and Short-Term
Interactive Application! You can complete this application by typing the information into the spaces provided on the form while it is displayed on your computer screen. After you are finished filling out the application please print it, sign it yourself and also have the application signed by the superintendent or superintendent's designee in your employing school district, prior to mailing the form to the address indicated on the application.
13. Initial License (education program completed inside of Ohio)
Interactive Application! You can complete this application by typing the information into the spaces provided on the form while it is displayed on your computer screen. After you are finished filling out the application please print it, sign it yourself and also have the application signed by your Ohio college or university official prior to mailing the form to the address indicated on the application.
This application may be used for the following purposes:
- An Ohio college or university recommending eligible individuals for an initial Ohio license, and to add new areas to existing licenses.
- An Ohio school district for persons being recommended for an initial career-technical license via the 24-hour program. The application must be accompanied by a CT-37 signed by the university in which the individual has enrolled in the 24-hour program.
- Individuals applying for an initial Physical Therapist, Occupational Therapist, Physical Therapist Assistant, Occupational Therapist Assistant license.
- Individuals who are applying for a new license earned through the Supplemental Pathway.
14. Initial License (education program completed outside of Ohio)
This application may be used by individuals who have completed licensure programs at colleges/universities outside the state of Ohio. Please read all of the instructions and information prior to completing and submitting the application. Please be sure to download the Instruction Packet for Out-of-State applicants. You may obtain the BCII and FBI criminal background report fingerprint cards by calling the Office of Educator Licensure at (614) 466-3593.
15. 5-Year License Renewal or Transition
This application may be used for the following purposes:
- To renew a five-year professional or associate license
- To transition a 4-year or 8-year certificate to a 5-year license
- To request a correction (e.g., name change) to a currently valid license, or to request a duplicate of a currently valid license.

This application is used to apply for educational aide permits and student monitor permits. Please note that applications for these permits need to be initiated by an employing Ohio school district.
18. Adult Education Teaching Permit
This permit is now available in either a one-year or four-year term length. Adult Education instructors can choose whichever permit they prefer.
This application may be used when requesting adult education teaching permits. These applications need to be initiated by the employing school district.
19. 5-Year School Treasurer and School Business Manager Licenses
This application may be used by individuals to apply for School Treasurer and School Business Manager licenses, new and renewal.
20. Non-Tax Certificate
This application may be used only by an employing Ohio private non-tax supported school to recommend individuals for a non-tax certificate. This application may not be submitted by individuals.
21. Pupil Activity Supervisor Permit

This application may be used by individuals to apply for the Pupil Activity Supervisor permit.
22. 2-Year License Renewal or Advance

Interactive Application! You can complete this application on the computer. When finished, print it, sign it and mail it to the address on the application.
This application may be used for the following purposes
- To renew a 2-year license
- To advance a 2-year provisional teaching, school counselor, or principal license to a 5-year professional license.

24. Conditional Teaching Permit
This application may be used to apply for a conditional teaching permit until December 31, 2010. An application for a conditional teaching permit needs to be initiated by an employing Ohio school district.
